<font>PROCEDURE FOR RE-ISSUING MEDICAL EXAMINATION AND TREATMENT DOCUMENTS</font>
<font>Legal basis</font>
<font>Law on Medical Examination and Treatment No. 15/2023/QH15 dated January 9, 2023 of the National Assembly of the Socialist Republic of Vietnam;</font>
<font>Circular No. 32/2023 dated December 31, 2023 of the Minister of Health detailing a number of articles of the Law on Medical Examination and Treatment ;</font>
<font>Circular No. 17/2012/TT-BYT dated October 24, 2012 of the Minister of Health; </font>
<font>Circular No. 56 / 2017 / TT -BYT dated December 29 , 2017 of the Minister of Health ;</font>
<font>Other current regulations of the Ministry of Health, Department of Health and Hospital.</font>
<font>Maximum 5 working days from the date of receipt of valid documents</font>
<font>Related documents</font>
<font>Application for re-issuance of medical examination and treatment documents (Applicable to patients/patient representatives) or Official dispatch / Letter of introduction (Applicable to agencies and organizations requesting the issuance of records)</font>
<font>Old documents have been issued </font><font>(Original for comparison or notarized copy).</font>
<font>Citizen ID card/Industry card of the person requesting re-issuance of documents.</font>
<font>Authorization letter certified by the People's Committee of the commune/ward where the patient resides or documents proving personal relationship (Birth certificate, Marriage certificate, Family register...) : in case the patient's representative requests re-issuance of documents.</font>
<font>Implementation sequence</font>
<font>For customers</font>
<font>Step 1 : Write </font><font>an application for re-issuance of Birth Certificate</font><font> according to form </font><font>BM.QĐ.HT-08.01</font><font> or </font><font>an application for re-issuance of documents related to medical examination and treatment according to form </font><font>BM.QĐ.HT-08.02</font><font> or an application for re-issuance of Death Certificate according to form </font><font>BM.QT.KHTH-31.06</font><font> or provide an Official Letter/Introduction Letter and related documents according to regulations. </font>
<font>Step 2 : Submit the documents directly at the hospital reception desk or send a scan of the documents - notarized in the file via email with the title: </font><font>Support for reissuance of documents related to medical examination and treatment.</font>
<font>Step 1 : Receptionist receives, checks documents and guides customers to pay fees according to regulations; sets a time to return results; sends documents to the General Planning Department</font>
<font>Step 2: The General Planning Department checks the validity of the documents and submits them to the Board of Directors for approval (If invalid/not approved: the General Planning Department saves the documents, transfers them to the receptionist for explanation and returns the fee to the customer). If valid and approved, the documents are transferred to the clinical department.</font>
<font>Step 3 : The clinical department issues documents, submits them to the Head/Deputy Head of Department and the Board of Directors for approval (if signature is required); sends a document stamped "Reissue" . Transfers the completed documents to the reception department.</font>
<font>Step 4. Receptionist returns results: in person, via email or express delivery.</font>
<font>Fees</font>
<font>150,000 VND</font>
<font>Note: </font>
<font>The hospital only reissues documents related to the medical examination and treatment process (Birth certificate; Hospital discharge certificate; Social insurance leave certificate; Maternity leave certificate; Surgery certificate or other documents related to the medical examination and treatment process such as: paraclinical results (tests, diagnostic imaging, functional examination); list of medical examination and treatment costs; ...) in the following cases:</font>
<font>Lost, broken;</font>
<font>The person signing the certificates is not authorized;</font>
<font>Stamping on certificates is not in accordance with regulations;</font>
<font>There are errors/mistakes/missing information recorded on the documents. </font>
<font>The patient or the patient's representative must honestly provide and be responsible for the information on the application form to the hospital and </font><font>must only use it for the purposes requested to the hospital.</font>
