<font>Reception</font>
<font>Receptionist (if the patient has been discharged from the hospital) or inpatient department administrative staff (if the patient is being treated) or doctor/nurse at the clinic (if the patient is being examined as an outpatient)</font>

<font>Related documents</font>
<font>Application for a certificate of injury (Applicable to the patient/patient's representative) or Official dispatch / Letter of introduction stating the request (Applicable to agencies and organizations requesting the provision of a certificate of injury).</font>
<font>Hospital discharge papers/Medical examination and treatment records </font><font>(Original for comparison or notarized copy).</font>
<font>Citizen ID card/Industry card of the person requesting the IT certificate.</font>
<font>Authorization letter certified by the People's Committee of the commune/ward where the patient resides or documents proving personal relationship (Birth certificate, Marriage certificate, Family register...) : in case the patient's representative requests an IT certificate.</font>
<font>Implementation sequence</font>
<font>For customers</font>
<font>Step 1 : </font><font>Write </font><font>an application for a certificate of injury </font><font>BM.QT-KHTH-34.01</font><font> or provide an Official Letter/Introduction Letter and related documents as prescribed. </font>
<font>Step 2 : Submit the documents directly at the hospital reception desk or send a scan of the documents - notarized in the file via email with the subject: </font><font>Request for a certificate of injury</font>
<font>Step 3: Pay the fee; receive an appointment letter (In case the patient is being treated as an inpatient, no fee is required)</font>
<font>For Hospitals</font>
<font>Step 1 : Receptionist receives, checks documents and guides customers to pay fees according to regulations; makes an appointment with customers for results; sends documents to the General Planning Department</font>
<font>Step 2: The General Planning Department checks the validity of the documents and submits them to the Board of Directors for approval (If invalid/not approved: The General Planning Department saves the documents, sends them to the receptionist for explanation and returns the fee to the customer). If valid and approved, the documents are transferred to the clinical department.</font>
<font>Step 3 : The clinical doctor checks the information and writes the Injury Certificate. Approves the Head/Deputy Head of Department and the Board of Directors. Transfers the completed file to the reception department.</font>
<font>Step 4. The receptionist returns the results in person, via email or express delivery.</font>
<font>Fees</font>
<font>150,000 VND</font>
<font>Note:</font>
<font>Injury certificates are only issued to patients with injuries who are admitted to the emergency room/treated at Hong Ngoc General Hospital - Phuc Truong Minh.</font>
<font>The certificate only provides information related to the process of admission, examination, treatment, and the patient's condition upon admission/discharge; the hospital does not perform injury assessment.</font>
